Add a column

At some point over the last few days, I seem to have lost the ability to add a column to my grade book. I press the plus symbol and column options come up for things like attendance, calculations etc but I want a simple column that I can add my own annotations to. This always used to be the default. Can you please tell me how I can go back to adding non-specific columns as my whole grade book is unusable without this.
