Grades don't save.....sometimes

I have this ongoing issue where I will enter grades in the seating plan screen, I will see the grade recorded, but every now and then some of them won't save when I leave the screen. Which can be annoying the next time I see the class and ask a student to complete the assessment and they say they've already done it. Anyone else have an issue with grades not saving?
